Get Back on Track After an Auto Accident or Workplace Injury

A car accident or a workplace injury brings its own kind of stress on top of the physical pain, between insurance calls, claim paperwork, and figuring out who to actually see for treatment. Our job is to make the physical therapy part of that simple: a thorough evaluation, a clear plan, and documentation that supports your claim.

At ProActive Physical Therapy and Wellness, we treat whiplash, back and neck strain, and other soft-tissue injuries from auto accidents and workplace incidents, with a plan built around your specific evaluation, designed by Dr. Brandon Evans, PT, DPT. Starting physical therapy early after this kind of injury is associated with a better recovery, not something to put off.

What to Know About PIP and Workers' Comp Physical Therapy in Kentucky

Kentucky is a no-fault state for auto accidents. Nearly every auto policy carries Personal Injury Protection, or PIP, coverage with a minimum of $10,000 in basic reparation benefits unless it was formally rejected in writing. Those benefits cover reasonable and necessary medical treatment, including physical therapy, regardless of who caused the accident. Workers' comp works differently: after a workplace injury, you typically see a designated treating physician first, and that physician directs referrals for further treatment, including physical therapy.

For general injuries outside of these two categories, Kentucky is a direct-access state and no referral is required to start physical therapy. For workers' comp specifically, care is coordinated through your designated physician, and we work directly with that physician's office to keep your treatment on track.

When You Should See a Doctor Before Starting Physical Therapy

Most soft-tissue injuries from an accident are safe to evaluate with physical therapy once you have been medically cleared. Seek immediate care first if you have severe or worsening pain, midline spine tenderness after significant trauma, any loss of consciousness or signs of a concussion, chest pain or shortness of breath, or new numbness or weakness. Those need emergency or physician evaluation before physical therapy. If any of that applies to you, please go to an emergency room or see your physician first, then come talk to us.

Is This a PIP Case or a Workers' Comp Case?

The two get confused since both involve insurance paying for treatment outside your regular health coverage, but they run through different systems. A PIP case comes from an auto accident, whether you were driving, a passenger, or a pedestrian, and is billed through the applicable auto insurance policy under Kentucky's no-fault system. A workers' comp case comes from an injury on the job and is billed through your employer's workers' compensation insurance, coordinated through your designated treating physician. Knowing which one applies determines how your care gets billed and documented, and we will help you sort that out on your first call.

What Makes This Different From a Regular Visit

Care itself looks the same as any other evaluation at ProActive: hands-on, one-on-one, and built around your specific injury. What is different is the billing and documentation. PIP and workers' comp claims are billed directly to the applicable auto or workers' comp insurer, not to your health insurance or Medicare. We require active PIP coverage or an accepted workers' comp claim to begin treatment on that basis, and we do not accept liens, meaning we do not defer payment until a case settles. We will tell you plainly during your first call whether your situation fits that model.

Physical therapy at ProActive includes:

  • A thorough evaluation to document your injury and build a plan around it
  • Manual therapy and a progressive exercise program targeted at your specific injury
  • Documentation that supports your PIP claim or workers' comp case
  • Coordination with your designated physician for workers' comp cases, or your physician for PIP cases when needed
  • Dry needling and other adjunct treatments when appropriate for your injury
  • Shockwave therapy for tendon-related overuse injuries from repetitive workplace tasks that have not fully responded to initial care

This is not:

  • Treatment on a lien, where payment is deferred until a case settles
  • A substitute for legal advice about your accident or workers' comp claim
  • Something we can start without active PIP coverage or an accepted workers' comp claim
  • A substitute for emergency or physician care when red flags are present

Built on Evidence-Based Care, Not Guesswork

Our approach to whiplash and other accident-related neck and back injuries follows the clinical practice guidelines published by the Orthopaedic Section of the American Physical Therapy Association, which specifically address whiplash-associated disorders. The guidelines support early education and reassurance about a good expected recovery, combined with manual therapy and a progressive exercise program, rather than prolonged rest. Starting appropriate care early, instead of waiting, is part of what supports a better outcome.

  • A thorough evaluation. We document your injury and build a plan around your specific evaluation findings.

Frequently Asked Questions

Do you accept liens for PIP or workers' comp cases?
No. We require active PIP coverage or an accepted workers' comp claim to begin treatment on that basis. We do not treat on a lien or defer payment until a case settles. We will confirm coverage status with you before your first visit.

Do I need a referral to start physical therapy after a car accident?
Not for most PIP cases. Kentucky is a direct-access state, so a referral is generally not required to start physical therapy after an auto accident, though we recommend confirming with your physician first if you have not yet been medically evaluated after the accident.

Do I need a referral for a workers' comp injury?
Generally, yes. Workers' comp care in Kentucky is coordinated through your designated treating physician, who directs referrals for further treatment, including physical therapy. We work directly with that physician's office.

What is PIP coverage?
Personal Injury Protection is required on nearly every Kentucky auto policy at a minimum of $10,000 in basic reparation benefits, unless formally rejected in writing. It covers reasonable and necessary medical treatment, including physical therapy, after an auto accident, regardless of who was at fault.

How soon after an accident should I start physical therapy?
Once you have been medically cleared, starting sooner rather than later is generally associated with a better recovery. Clinical guidelines for whiplash and accident-related neck and back injuries support early movement and exercise over prolonged rest.

What if I am not sure whether my case is PIP or workers' comp?
That is common, and we will help you sort it out. Call or text us with the basics of what happened and we will help you understand which applies to your situation.

How do I get started?
Call or text us at (502) 512-2165, or email info@proactiveptlou.com. We will talk through what happened, confirm your coverage, and figure out together whether physical therapy is the right next step for you.
