It can be very difficult to navigate how to stay active during pregnancy, how to best prepare your body for an optimal pregnancy or how to handle potential postpartum issues.

Pregnancy

It is very common during pregnancy to experience hip and low back pain, urinary incontinence and/or pelvic floor dysfunction/pain. As your body prepares for the birthing process, it starts to release hormones that cause your ligaments and tendons to relax so that your baby can travel safely through the canal. As these ligaments and tendons relax, your joints around your pelvic area can become unstable leading to pain, discomfort and/or urinary incontinence. Also during pregnancy many of your core muscles become overstretched and weak as the baby grows inside your womb exacerbating the instability in your pelvic and low back region. The combination of laxity in the ligaments/tendons and the weakening of the core muscles can be a leading cause of pain and discomfort.

Another common issue that is resultant laxity and weakness is urinary incontinence. Note: We understand that this is a very sensitive topic but one that needs to be addressed as so many women deal with this but hesitate to let their healthcare professional know.

The urethra is the tube-like structure that allows urine to travel from your bladder to be expelled outside your body. At the top and bottom of the urethra are two sphincters which help open and close the urethra to allow urine to pass from your bladder to outside of your body. There are muscles that are located in your pelvic floor that help to open and close these sphincters which ultimately allow or stop the flow of urine. Think of these muscles as the “gatekeepers”of your urine. So when these muscles contract they close the sphincters and stop the flow of urine and when they relax they allow the sphincters to open and urine to flow. If these muscles are weak, the sphincters never fully close which ultimately causes urinary incontinence or “leaking”. As you progress through your pregnancy these muscles can become even weaker which can lead to “leaking” or urinary incontinence.

Postpartum

The delivery process plays a great toll on your core and pelvic floor muscles. As the baby is exiting the canal, ripping and tearing typically occur in your core and pelvic floor musculature. In the case of a C-section, even more damage occurs as all of your abdominal core musculature has to be cut through to deliver your beautiful baby. Another thing to remember is that after a C-section, scar tissue can build up around the incision site which can also cause recovery complications.

After delivery, many women never receive any instruction on how to restrengthen these muscles that were stretched and torn during the delivery process. The story that we typically hear is “I was told to go home and rest”. Yes rest is very important and will help your body start to heal, but if you never restrengthen your core and pelvic floor muscles, you increase the likelihood of having pelvic floor dysfunction, pain with sex, low back pain and/or urinary incontinence months (and even years) after the birth of your child. Note that the more children you have, the increased likelihood that you will deal with one or more of these problems.

That’s why is so important to PROPERLY restrengthen your core and pelvic floor muscles postpartum to ensure you can live the active and healthy lifestyle that you deserve without low back or hip pain, fear of “leaking”, pain with intercourse, diastasis recti, or pelvic floor dysfunction.

I will let you in on a little secret…Kegels alone are NOT the solution to fix this problem.

After a total knee replacement 10 months ago, I was still dealing with lingering pain and limited function of that knee (and feeling pretty depressed).
