Dr. Brandon Evans, PT, DPT

I started this clinic because of my grandmother. In 2008, she lost her life to cancer that wasn't caught in time — years of falls and health concerns were dismissed as "just getting older" until it was too late. Watching someone I loved be failed by that mindset changed how I think about care. ProActive exists so no one has to accept decline as inevitable, and so "that's just aging" is never the end of the conversation.

Every plan I build starts with one question: what does this person actually want to get back to? Not a generic range-of-motion goal — their golf swing, their grandkids, their ability to travel without dreading the flight.

Do I need a doctor's referral to start physical therapy?
In most cases, no. Kentucky is a direct-access state, so you can generally start physical therapy without a referral. Medicare and workers' comp have their own specific requirements, which we'll walk you through on your first call.

Does ProActive accept Medicare?
Yes, we accept Traditional Medicare Part B. Many of our patients choose ProActive specifically for one-on-one, unhurried care that goes beyond what a typical insurance-driven visit allows — and we're glad to work with Medicare patients as well.

I was dealing with shoulder pain for years and I couldn't do any press movements. After the first couple of visits, the pain and range of motion had gotten noticeably better! As we continued I started to get back to doing my usual workouts in the gym and was able to be consistent with no pain after. He was very knowledgeable and detailed about what we had to do to for to get better. I would recommend them to anybody dealing shoulder pains.

I have been to Physical Therapy before, but never experienced anything close to what I got at Pro Active. Individual care, friendly caring people and they never pushed you past your limit. Brandon actually took my call (on a Saturday) for half an hour to help me with a particular problem. Wish I could continue but the three I worked with did too good a job and I no longer need them.

I have been going to physical therapy for the past few months to treat muscle pain and aches after a car accident, and I have had a very positive experience working with Lauren.

Lauren is very thorough and thoughtful in her treatment approach. She takes things step by step and has helped address my neck muscle aches, wrist pain, and other symptoms in a way that feels careful and individualized. What I appreciate most is that she does not rush the recovery process or try to discharge me too early. She makes sure I am progressing safely, checks for any flare ups, and adjusts my treatment as needed.

She also gives home physical therapy exercises that are tailored to my specific symptoms, which has helped me continue improving outside of each session. I really appreciate her patience, professionalism, and commitment to making sure I fully recover.

2 years out from having a glute tear and repair I feel like I’m finally getting the physical therapy that I really need. I’ve had PT before but I feel that I’m getting much more in-depth therapy at ProActive.
